Please note this form is to apply for regular two wheeled bikes. If your school requires adaptive bikes or trikes, please contact info@varietytas.org.au

Please tells us how many bikes you are requesting for each size.

Sizes are by child height.

Where possible we aim to have bikes in neutral colours, subject to the stock availability at the time. However, we can't guarantee that any child will receive their preferred colour. 

Please write how many helmets are requested for each size.

Sizes are by head circumference.

If I am successful with this application, I acknowledge and grant “Variety –the Children’s Charity Tasmania” the permission – or have the permission of the parent or guardian of the child/ren that I have requested a bike for - to create and use:
• photographs, video and/or audio recordings of my child/ren, and
• in some instances, quotes and – if appropriate - my child’s/children’s first name/s as
well as to use the above mentioned for non-for-profit, non-commercial purposes
to be distributed in locations like:
• printed publications e.g. newsletters, annual reports, promotional material
• online publications e.g. on secure intranet websites and publicly accessible Variety
Tasmania’s and Variety Tasmania’s partner websites, Variety Tasmania’s social media
platforms e.g. Facebook, LinkedIn, Instagram, Twitter and • other forms of media e.g. podcasts.
I give or have been given this permission by the guardian/s of the child/ren without myself
or the guardian/s expectations of compensation.
The permission will continue until you revoke permission in writing to
ceo@varietytas.org.au. Where permission is revoked, every effort will be made to
remove relevant media from distribution, however this may not be possible or practical in
some situations.

Your privacy is respected by Variety. The personal information you provide on this form
will be used to assess your eligibility. It may be provided to organisations who assist us, or
as required by authorised by law, but we will not use any of your sensitive information for
marketing purposes without your prior consent.
