Eligibility criteria:

• Child or children reside in WA
• Child or children are aged 4-17 (must be under 18 at time of submission)
• Child or children are enrolled and attending (or will be attending) Primary or High School in 2022.
• Child or children who are disadvantaged (financial, geographic, social), have a disability or long-term health
• The family household income is below the guide*
• Equipment is not available via government or alternative sources

Application:

• Application form can be complete for one or more children in your primary care that meet the eligibility.
• An application can be completed by a parent/carer, or approved third party (Key Therapist, Social Worker, Support Worker etc.) however all information must be approved/submitted by parent/carer.
• Applicants will be expected to submit a truthful and factual application and attach relevant documentation as evidence where requested.
• • Income details are required (this includes any/all income received to confirm annual household income)
  • • Payslips – FT / PT work (dated within 3 months)
    • Centrelink statements (dated within 3 months)
    • valid Healthcare card
    • 2020-21 ATO notice of assessment (for self-employed)
  • Quotes for equipment / items
  • • Quotes required for each items requested (can source multiple items from one supplier)
    • Technology quotes to be sourced from a Variety preferred supplier (unless otherwise mandated by School).

• Quotes must be no older than three months at the time of submission and must remain valid throughout the assessment

• Supporting document(s) from Schools (must be at least one)
• • Booklists
  • Forms listing mandatory requirements / items
  • Supporting letter from teacher / principal

Review: 

• Submissions will be reviewed by the Kids Support Committee
• The Committee will host regular meetings to review applications. Submission date will dictate meeting date.
• The committee will not be reviewing application between 20^th Dec 2021 - 10^th Jan 2022.
• Variety requests that you allow up to 2-3  weeks from submission date for an outcome notification (bearing in mind above-mentioned pause).
• Variety aims to have all submissions reviewed and applicants notified by no later than Monday 24th January 2022.

Important info:

• Retrospective funding is not eligible – ONLY items that have been purchase post approval date will be accepted.
• Funding can only be accessed via Tax Invoice from supplier or reimbursement upon proof of purchase of approved items ONLY.
• Applications will be reviewed regularly, so please ensure you submit as soon as possible.
• The round will close on Tuesday 11th January 2022, there will not be any extensions given.
