Welcome the Variety NSW/ACT Feeds & Nappies Application Form

Variety NSW/ACT provides assistance (via reimbursement only) towards the purchase of nutritional feeds (including associated consumables) and nappies to assist children that have a medically diagnosed chronic illness or disability.

Eligibility

• Applicants must be Australian Citizens or Permanent Residents
• Applicants must live in NSW or ACT
• Applications can only be accepted for children aged 0 to 18 years
• Applicants must be medically diagnosed with a chronic illness or disability
• Previous recipients will not be considered for another a Nutritional Feeds & Nappies grant until 12 months after the date listed on their outcome letter
• Unsuccessful applicants may be eligible to reapply for a Nutritional Feeds & Nappies grant 12 months after the date listed on their outcome letter.

Feeds and Nappies Requests

• Nutritional Feeds (oral nutritional supplements, formula, food thickener etc.)
• Consumables (feeding sets, enteral dispensers/syringes, enteral feeding tubes/devices and accessories, wipes etc.)
• Nappies

Documentation Required 

Variety NSW/ACT recommends consulting with your NDIS planner, GP or Specialist before starting an application so they can best support you through the process.

Some documentation will be required for your application depending on your situation, these may include support letters, diagnosis letters & proof of income. Please check the guidelines.
