- You can download the form, fill it in by hand, and email it to us
- You can create a video submission and send to our team
If you choose to submit a video, please make sure your responses address the key application questions.
Supporting Documentation
Please note that supporting documentation, such as diagnosis letters, income statements and referral letters are required and must be submitted as part of your application.
